Abstract

Hamstring flexibility is an important factor that affects muscle performance of the lower extremities and is closely associated with sports injuries. Therefore, evaluation of flexibility is important in clinical practice. Results of evaluation are determined by types of tests and cut-off values used; therefore, accurate and detailed understanding of these is necessary before examination. Although the straight leg raise and sit and reach tests are used to evaluate hamstring extensibility, structures including the nerves, fascia, and other muscles can significantly confound the results of these tests. The knee extension test is performed at 90° of hip flexion to minimize the posterior pelvic tilt that occurs during the straight leg test. The knee extension test is most recommended for selective evaluation of hamstring flexibility. The knee extension test is classified into active and passive tests. The cut-off value is usually set at 20° for the active and at 10° for the passive knee extension test. Although a strong association is observed between the two tests, the active knee extension test is preferred in clinical practice because it can be performed by a single examiner, which serves as an advantage. Age, sex, and warm-up exercise tend to affect flexibility; therefore, results should be interpreted with caution. Detailed understanding of each flexibility test is important for reliable evaluation.

Ⅰ. 서 론

하지 근육 중 넓적다리뒤근육(hamstring muscle)은 단축 시 빠른 속도의 움직임이 수반되는 럭비, 야구, 축구 등에서 스포츠 손상 위험도가 증가한다 (Croisier 2004; Ekstrand 등, 2011; Henderson 등, 2010; Witvrouw 등, 2003). 넓적다리뒤근육의 짧아진 길이는 볼기근 약화(gluteal muscle weakness)와 밀접하게 관련이 있으며, 엉치엉덩관절(sacroiliac joint)의 안정성을 떨어트릴 수 있다 (Massoud Arab 등, 2011). 이 때문에 임상에서는 넓적다리뒤근육의 유연성 검사 결과를 재활치료 프로그램의 단계별 시행 및 현장 복귀를 위한 기준(criteria)으로 활용하고 있다(Heiderscheit 등, 2010; Whiteley 등, 2018).

현재 넓적다리뒤근육의 단축 여부를 평가하기 위해 하지직거상(strait leg raise) 검사, 앉아 윗몸 앞으로 굽히기(sit and reach) 검사, 무릎폄(knee extension) 검사 등 3가지가 대표적으로 사용되고 있다. 하지직거상과 앉아 윗몸 앞으로 굽히기 검사는 오랜 기간 임상에서 널리 사용됐지만, 엉덩관절의 움직임이 아래로는 하지 근육의 신장성(extensibility)과 위로는 체간 근육의 신장성에 영향을 받는 단점을 갖고 있다. 하지직거상 동안 엉덩관절 1.7° 굽힘에 비례하여 골반은 1°의 후방 회전이 발생하고, 앉아 윗몸 앞으로 굽히기에서 발목관절의 발바닥 굽힘 허용 여부는 결괏값에 약 3cm 정도 영향을 미치는 것으로 확인된다 (Bohannon 등, 1985; Liemohn 등, 1997). 넓적다리 뒤근육의 분리된 검사를 위해 Gajdosik과 Lusin(1983)은 무릎폄 검사를 제시하였다 (Gajdosik과 Lusin 1983). 무릎 폄 검사는 엉덩관절 90° 굽힘 상태에서 수행하기 때문에 하지직거상으로 인해 발생하는 불필요한 골반의 회전을 최소화할 수 있다. 즉, 하지직거상과 달리 더 선택적으로 넓적다리뒤근육의 신장을 유도할 수 있다. 현재 무릎폄 검사는 연구 목적뿐만 아니라 임상 현장에서도 널리 사용되고 있다. 하지만 같은 대상자도 수행 절차에 따라 평가가 다를 수 있고 다양한 기준값이 현재 혼재되어 사용되고 있기 때문에, 임상에서는 이에 대한 심도 있는 이해가 필요하다. 또한, 나이 및 성별, 사전 준비 운동 여부, 허리통증 여부가 결괏값에 영향을 유의하게 미치기 때문에 주의가 필요하다.

본 종설은 올바른 넓적다리뒤근육의 유연성 검사 및 평가를 돕고자, 기존 선행 연구를 바탕으로 무릎폄 검사를 분류하고, 검사별 수행 절차, 단축 평가 기준, 신뢰도 및 타당도를 제시하였다. 그리고 일부 논의가 필요한 사항은 별도로 정리하였다.


Ⅱ. 무릎폄 검사
1. 검사 분류

무릎폄 검사는 피험자의 자립적 수행 여부에 따라 능동과 수동으로 구분된다. 초기에는 피험자의 능동적 움직임에 의해 수행되는 무릎폄 검사만이 사용되었다. 하지만 Fredriksen 등(1997)은 능동 무릎폄의 최대 각도가 넓적다리뒤근의 신장성과 완화(relaxation)뿐만 아니라, 넓적다리네갈래근(quadriceps femoris muscle)의 근력에도 영향을 받을 수 있어서 수동 무릎폄 검사가 더 나은 선택이 될 수 있다고 주장하였다 (Fredriksen 등, 1997). 능동 무릎 폄을 수행하는 넓적다리네갈래근의 근력이 매우 크다면 마지막 지점에서 수동 저항을 이겨내고 추가적 무릎폄이 일정 부분 가능하다는 것이다. 하지만 이에 대한 과학적 근거를 제시하지는 못했다. 최근 넓적다리네갈래근의 영향을 확인하기 위해, 남녀를 구분하여 넓적다리네갈래근의 근력과 능동 무릎폄 검사와의 상관관계를 확인한 연구가 있었다. 실험 결과 남녀 모두 두 변수 간 유의한 관계가 관찰되지 않았으며, 근력에 차이가 있는 우세다리(dominant leg)와 비우세다리(non-dominant leg) 간 무릎폄 각도에서도 차이가 발견되지 않았다 (Yıldırım 등, 2018). 즉, 능동 무릎폄의 최대 각도는 넓적다리네갈래근의 근력에 큰 영향을 받지 않는 것으로 여겨진다. 또한, 많은 연구에서 능동과 수동 무릎폄 검사 간 상당히 높은 상관관계가 확인되고 있으므로, 유연성 평가 시 둘 모두를 기계적으로 수행하기보다는 상황 및 여건에 따라 하나를 선택적으로 수행하는 것이 효율적일 것이다. 예를 들어, 수동 검사는 치료사 2명(수동적 무릎관절을 폄을 수행하는 치료사 1명과 마지막 지점에서 무릎폄 각도를 측정하는 치료사 1명)이 필요하므로, 임상에서는 능동 무릎폄 검사가 나은 선택이 될 수 있다. 만약 특정 사유로 두 검사를 모두 수행해야 한다면, 능동 검사는 넓적다리뒤근육의 상반억제(reciprocal inhibition)를 유발할 수 있어서 수동 검사를 먼저 수행하는 것이 추천된다 (Agrawal 2016).

2. 수행 절차

피험자는 치료 테이블 위에 바로누운자세(supine position)로 눕는다. 엉덩관절과 무릎관절은 완전 폄 후 측정반대 측 다리(contralateral leg)와 골반은 스트랩을 이용하여 치료 테이블 위에 고정한다 (Figure 1a). 피험자는 동측 다리(ipsilateral leg)의 엉덩관절을 90° 굽힘 후 무릎관절 폄을 천천히 수행한다. 한 가지 주의할 점은 무릎폄 동안 엉덩관절은 90° 굽힘을 유지해야 한다는 것이다. 엉덩관절 90° 굽힘 유지를 위해 넓적다리(thigh)는 지면과 지속해서 수직을 이루고 있어야 하는데, 도움 없이 피험자의 노력만으로 이를 안정적으로 유지하는 것은 어렵다. 일반적으로 보조물을 다리 아래 두어 엉덩관절 굽힘 유지를 돕는다 (Neto 등, 2015; Oh 등, 2019; Youdas 등, 2010) (Figure 1b). 능동 무릎폄 시 피험자는 근간대경련(myoclonus)이 나타날 때까지 최대 무릎폄을 수행하며, 최대 폄 도달 시 무릎관절을 조금 굽혀 근간대경련이 사라지도록 하고 이를 마지막 지점(endpoint)으로 정한다 (Gajdosik과 Lusin 1983; Yıldırım 등, 2018) (Figure 1c). 일부 연구에서는 첫 번째 스트레칭 감각(first stretch sensation)이 느껴지는 지점을 마지막 지점으로 정하기도 한다 (Cameron과 Bohannon 1993; Corkery 등, 2007; Kuilart 등, 2005; Turl과 George 1998). 수동 무릎폄의 경우 피험자의 불편(discomfort) 또는 통증 반응과 치료사가 느끼는 근 힘줄단위(muscle tendon unit)에서의 저항 수준을 종합적으로 고려하여 마지막 지점을 결정한다.

무릎폄 각도는 넓적다리가쪽위관절융기(lateral epicondyle of the femur)를 축으로 넓적다리큰돌기(greater trochanter of the femur)와 종아리가쪽복사(lateral malleolus of the fibula) 사이의 각을 측정한다. 각도계(goniometer), 전자 경사측정기(digital inclinometer)(Figure 2a). 를 주로 사용하며 (Davis 등, 2008; Neto 등, 2015; Youdas 등, 2010), 최근에는 관성 측정장치(inertial measurement unit)(Figure 2b) 및 스마트폰(smartphone)(Figure 2c)의 활용도가 점차 높아지고 있다 (Dos Santos 등, 2017; Gnat 등, 2010; Oh 등, 2019). 신뢰도 평가와 같은 특정한 목적이 있는 경우가 아니라면, 측정은 주로 2회 또는 3회를 수행하며 평균값을 사용한다 (Hamid 등, 2013; Neto 등, 2015; Norris와 Matthews 2005).

3. 평가 기준

검사 값은 측정한 ‘무릎폄 각도(knee extension angle)’(Magalhães 등, 2015; Youdas 등, 2010)를 추가적 계산 없이 그대로 사용하기도 하며, ‘정강이뼈와 수평선 간 사이 각’(Nakaizumi 등, 2018), 또는 ‘180-무릎폄 각도’(Figure 1c)(Kuilart 등, 2005; Lim 2020; Lim 2019; Park과 Lim 2020)로 재정의하여 사용하기도 한다. 현재 마지막 방법을 가장 널리 사용하고 있으며, 평가를 위한 절단값(cut-off value)은 종종 20°로 정하고 있다. 검사 값이 20°보다 크면 근육에 단축이 있다고 판단한다 (Singh 등, 2017; Youdas 등, 2010). 넓적다리뒤근육의 단축을 평가하는 기준(inclusion criteria)으로‘능동 무릎폄 각도 > 20°’에 사실 광범위한 동의나 합의는 없으며, 이에 대한 과학적 근거도 불분명하다. 일부 연구에서는 절단값으로 30°(Bandy 등, 1997; Erkula 등, 2002; Johnson 등, 2014; Neto 등, 2015), 25°(Magalhães 등, 2015), 15°(Kuilart 등, 2005; Webright 등, 1997)등 다른 기준을 사용하기도 한다. 수동 관절폄각도가 능동 관절폄 각도 보다 더 크기 때문에 (Oh 등, 2019), 수동 무릎폄 검사 시에는 절단값을 10°로 낮추어 정하기도 한다 (Gnat 등, 2010).

동일 검사 내 기준값이 상이하게 제시되는 것은 실험마다 조건 및 상황이 다르기 때문일 것이다. 실험 대상자의 연령대가 다를 수 있고, 남녀 비율이 다를 수 있다. 또한, 신체 특성이 같다 하더라도, 실험설계 시 마지막 범위를 어떻게 정의하는지에 따라서도 결과는 다르게 나타날 수 있다 (Yıldırım 등, 2018).

4. 신뢰도 및 타당도

건강한 성인을 대상으로 한 능동 무릎폄 검사의 측정자 내 신뢰도(intrarater reliability)는 0.93~0.95로 매우 높았다 (Davis 등, 2008; Oh 등, 2019). 양다리를 나누어 평가하더라도 우세다리 0.92, 비우세다리 0.88로 큰 차이가 없었다 (Hamid 등, 2013). 만약 넓적다리뒤근에 단축이 있다면 엉덩관절 굽힘 동안 골반 움직임이 과도하게 발생할 수 있어, 반복 측정 시 상대적으로 변동이 커질 수 있다. 하지만, 절단값을 30°로 설정하여 일정 수준의 단축이 있는 피험자를 대상으로 진행한 연구에서 0.91(95% confidence interval, 0.87-0.94)로 단축 여부와 관계없이, 무릎폄 검사는 높은 신뢰도를 보였다 (Neto 등, 2015). 무릎폄 검사는 대부분 능동 형태로 수행되기 때문에, 수동 무릎폄 검사에 관한 신뢰도 연구는 많지 않다. 대상자가 14명으로 많지는 않지만, 건강한 성인을 대상으로 한 Gnat 등(2010)의 연구에서 수동 무릎폄의 측정자 내 신뢰도는 0.88~0.93으로 높았다 (Gnat 등, 2010).

무릎폄 검사와 하지직거상 또는 앉아 윗몸 앞으로 굽히기 검사의 상관관계는 높지 않았다 (Davis 등, 2008; Hansberger 등, 2019). 검사 방법에 따라 측정 자세, 골반 위치, 신경 조직에 발생하는 수동 장력, 대상 인체 분절(segment)의 길이 및 비수축성 조직(noncontractile tissue)의 특성 등이 다를 수 있어, 검사 간 상관관계는 대체로 낮았다 (Cameron과 Bohannon 1993; Gajdosik 2001; Kendall 등, 2005). 각 검사 방법은 고유의 특성과 한계를 갖고 있으므로, 치료사는 선택에 있어 종합적 판단이 필요하다.


Ⅲ. 그 밖의 검사
1. 하지직거상 검사 (straight leg raise test)

하지직거상 검사는 가장 널리 사용되는 평가 방법의 하나로, 무릎폄 검사와 함께 넓적다리뒤근육의 단축을 평가하기 위한 표준(gold standard)으로 받아들여지고 있다. 피험자의 자발적 움직임에 기초한 능동 검사를 기본으로 하는 무릎폄 검사와 달리, 하지직거상 검사는 수동 형태로 수행되는 것이 일반적이다. 수동 하지직거상 검사는 불필요한 넓적다리네갈래근과 엉덩관절 굽힘근의 참여를 제거할 수 있는 장점이 있다고 알려져 있다 (Ayala 등, 2013). 능동 하지직거상의 경우 수동 형태의 이점을 갖지 못하기 때문에, 능동 무릎폄 검사 대비 사용 빈도가 낮다. 다만, 건강한 성인은 반대 측 넓적다리두갈래근(biceps femoris muscle)의 활성화가 하지직거상 중 발생하는 골반 앞 기울임을 일부 상쇄하여 능동 수축으로 말미암은 부정적 영향을 일부 줄일 수 있다 (Hu 등, 2012). 하지직거상의 최대 범위는 근육의 단축 외 근막(fascia), 신경 조직(neural tissues), 관절 주머니(joint capsule) 등에 의해서도 영향을 받는다 (Lim 2021a; Miyamoto 등, 2018). 또한, 골반 위치와 반대 측 엉덩관절 굽힘근의 단축도 엉덩 관절 굽힘을 제한하는 것으로 알려져 있다 (Bohannon 등, 1985; Fredriksen 등, 1997). 즉, 만약 평가의 목적이 다른 구조물의 영향은 최소화하고 넓적다리뒤근육의 신장성을 보다 중점적으로 확인하는 것이라면, 하지직거상 검사가 가장 우선하여 추천되지는 않을 것이다. 절단값(cut-off value)은 80°로 측정값이 80°보다 작은 경우 단축이 있다고 판단한다 (Castellote-Caballero 등, 2014; Gajdosik 등, 1992; Kendall 등, 2005). 건강한 성인을 대상으로 한 측정자 내 신뢰도는 0.92~0.95로 높은 수준이며 (Aalto 등, 2005; Davis 등, 2008), 단축 대상자에서도 0.95~0.96(95% confidence interval, 0.93~0.97)로 높았으며, 우세다리와 비우세다리 간 유의한 차이는 없었다 (Neto 등, 2015).

2. 앉아 윗몸 앞으로 굽히기 검사 (Sit and reach test)

메타 분석에 따르면, 앉아 윗몸 앞으로 굽히기 검사는 허리 신장성(lumbar extensibility)보다는 넓적다리뒤근육의 신장성 측정에 적합하며, 수정된 검사 (modified sit and reach test)보다는 고전적 검사 (classic sit and reach test)가 더욱 추정에 유리하다고 판단된다 (Mayorga-Vega 등, 2014). 하지만 발목관절의 발등굽힘 동작으로 발생한 신경 조직의 신장이 체간의 전방 굽힘을 방해하기 때문에 넓적다리뒤근육의 신장을 저해할 수 있다 (Gajdosik 등, 1985; Liemohn 등, 1997). 실제 최근 전문화된 측정 도구를 통한 연구에서 가동 범위는 넓적다리뒤근육 강직(hamstring stiffness)보다 다른 요소에 더 크게 영향을 받는 것으로 확인되었다 (Miyamoto 등, 2018). Kendall 등(2005)은 앉아 윗몸 앞으로 굽히기 검사에서의 교란 변수(confounding variables)의 영향을 최소화하기 위해 수정된 검사(modified sit and reach test)를 제안하기도 했다 (Kendall 등, 2005). 앉아 윗몸 앞으로 굽히기 검사는 무릎폄검사를 수행할 수 없는 환경에서 대안적으로 사용 가능할 수 있으나, 다른 변수들 때문에 독립적 넓적다리뒤근육의 신장성 측정을 위해서는 제안되지 않는다. 측정자 내 신뢰도는 0.94로 다른 검사와 같이 높은 수준을 보였다(Davis 등, 2008).


Ⅳ. 논 의
1. 나이 및 성별

신체의 나이가 증가하면, 수축성 조직은 감소하고 비수축성 조직은 증가하게 된다. 이에 따라 관절 유연성은 점진적으로 감소한다 (Bell과 Hoshizaki 1981). 단일 관절의 움직임뿐만 아니라, 허리골반 움직임(lumbopelvic motion)과 같은 복합적 움직임도 노화에 영향을 받는 것으로 확인되고 있다 (Zawadka 등, 2018).

일반적으로 여성은 남성보다 큰 관절 유연성을 갖고 있다 (Bell과 Hoshizaki 1981). 건강한 성인을 대상으로한 실험에서, 남녀 간 능동 무릎폄 검사는 7.3°, 수동 무릎폄 검사는 4.4° 차이가 있었다 (Yıldırım 등, 2018). 남성은 능동과 수동 무릎폄 검사 간 차이가 거의 없었지만, 여성은 두 검사 간 차이가 컸다. 무릎폄 검사와 마찬가지로, 수동 하지직거상에서도 여성은 8° 더 유연함을 보였다 (Youdas 등, 2005). 유연성 변동에 대한 정확한 평가를 위해서는 남녀를 별도 그룹으로 분리 후 분석하는 것이 필요하며, 차후 나이와 함께 성별에 따른 기준이 새롭게 정립되어야 할 것이다.

2. 사전 준비 운동(warm-up exercise) 여부

유연성 측정 시 사전 준비 운동(warm-up) 여부가 결과에 영향을 미칠 수 있다. 사전 준비 운동은 근육 피로를 일으키지 않는 범위 내에서 시행하며, 에르고미터(ergometer) 또는 트레드밀(treadmill)을 활용하여 가벼운 운동을 수행하는 것이 가장 보편화된 방법이다 (Yıldırım 등, 2018). Atha 등(1976)의 연구 이후 일부에서는 사전 준비 운동 목적으로 신체 전반을 움직이기보다는 대상 관절의 굽힘-폄만을 반복적으로 수행하기도 한다 (Atha와 Wheatley 1976). 능동 무릎폄 검사에서는 무릎관절 폄-굽힘 동작을 6회 반복 수행하며, 마지막 측정 각도를 기준 값(baseline)으로 사용한다 (Depino 등, 2000; Lim 2019; Spernoga 등, 2001). 하지만 관성 측정장치를 활용하여 6회 동안의 능동 무릎폄 각도를 실시간으로 측정한 연구에서 유연성은 예상과 달리 감소하였으며, 높은 변이 탓에 측정 간 신뢰도 역시 낮게 보고되었다 (Lim 2021b). 가벼운 운동을 수행하는 전통적 방법이 아닌, 무릎관절의 반복적 굽힘-폄을 활용하는 대안적 방법은 사전 준비 운동으로 사용 시 주의가 필요하다.

3. 허리통증(low back pain) 유무

감소한 넓적다리뒤근육의 유연성이 허리통증 위험성을 증가시킨다는 일부 연구가 있다. 하지만 최근 메타 분석에서는 기존 연구 결과를 뒷받침하는 근거의 질적 수준이 낮으며, 이에 따라 허리통증 환자가 손상된 넓적다리 뒤근육의 유연성을 갖고 있다고 단정적으로 결론지을 수 없다고 판단하였다 (Hori 등, 2021). 참고로 허리통증 환자를 대상으로 수행한 능동 무릎폄 검사와 하지직거상 검사는 매우 낮은 수준의 유의한 상관관계가 관찰되었다 (Shamsi 2019).


Ⅴ. 결 론

넓적다리뒤근육 단축은 근 수행능력 저하 및 운동 중 손상 위험도 증가와 연계되어 있기 때문에, 임상에서 넓적다리뒤근육의 유연성 평가는 중요하게 고려된다. 현재 하지직거상 검사, 앉아 윗몸 앞으로 굽히기 검사, 무릎폄 검사 등이 대표적으로 사용되고 있다. 각 검사는 고유의 특징을 갖고 있고, 어느 하나가 절대적으로 우월하지 않기에 상황 및 여건에 따라 선택되는 것이 적합하다. 다만, 하지직거상 검사와 앉아 윗몸 앞으로 굽히기 검사는 넓적다리뒤근육 외 다른 조직에 영향을 크게 받는 것으로 확인되고 있으므로, 보다 선택적으로 넓적다리뒤근육의 유연성을 검사하는 것이 목적이라면 무릎폄 검사가 가장 적합할 것이다. 무릎폄 검사는 능동과 수동으로 구분되는데, 능동 검사는 단일 측정자가 수행 가능하여 효율성 측면에서 추가적인 이점을 갖는다. 마지막으로 넓적다리뒤근육의 유연성은 나이, 성별, 사전 준비 운동 여부 등에 따라 영향을 받기 때문에, 해석 시 이를 면밀히 검토해야 한다. 신뢰할 수 있는 평가를 위해서는 이에 대한 세부적인 이해가 선행되어야 할 것이다.
